 Train To Go Academy Coach/Trainer: Coach Chris Location: 391 W. Water St, Taunton, MA 02780 1. Acknowledgment of Risk I, the undersigned participant (or parent/guardian if under 18), acknowledge that participation in athletic training, conditioning, and related activities involves inherent risks. These may include, but are not limited to, physical injury, muscle strains, sprains, fractures, paralysis, or even death. I understand these risks and voluntarily choose to participate. 2. Assumption of Risk I knowingly and freely assume all such risks, both known and unknown, and take full responsibility for my participation in all training sessions, programs, and events organized by Train To Go Academy or Coach Chris. 3. Release and Waiver In consideration of being allowed to participate in any training session, I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, release, waive, and hold harmless Train To Go Academy, Coach Chris, their employees, volunteers, agents, and representatives from any and all claims, demands, losses, or liabilities arising out of or related to any injury, disability, or loss I may suffer while participating in training activities, whether caused by negligence or otherwise. 4. Medical Treatment I authorize Train To Go Academy and its representatives to obtain emergency medical care if necessary and agree to be responsible for any costs incurred as a result. 5. Understanding and Agreement I have read this release of liability and assumption of risk agreement, fully understand its terms, and sign it freely and voluntarily. I understand that by signing this document, I am giving up substantial legal rights.
